Resolved hanging paragraphs. As I anticipated, this was one of the hardest comments to resolve. As a general principle, I resolved hanging paragraphs, whenever possible, by promoting sub-headings. There were also cases where there was only one sub-heading, such as 13.5.22 <form:grid> which was followed by 13.5.22.1 <form:column>, which isn't a valid outline. I simply promoted the sub-headings in those cases. The other reason for the resolution of hanging paragraphs was to make whatever semantics exist between elements explicit and not implied by grouping. Having said that, there were cases, see 10.5 3D Shapes, which are legitimate groupings of similar items. But there isn't any semantic other than being of the same type.
