Labelling of headers and section numbers as per the AMQP WG document...

      We need headers and sections such as in the AMQP WG doc so that we know where we are in the the document....

      That we do what was done in the AMQP WG document.

      i..e. for the headings....
      "Part 1 - Types - Type Encodings" as an example in the top right heading

      and for the section numbering....
      "1.2.1 Fixed Width" as an example of a section number where the first number is the "Part" or "Book" number.

      It is very easy to end up in the wrong section of the document as headers and section do not clearly label the "part" number of the standard.
