Improve presentation of the Revision History of the document

Currently the revision history is generated solely from the list of JIRAs which does not include the date of release of any given document revision, nor does it contain document revisions which did not include any JIRAs (such as the Public Review Draft versions).

Update the revision history generation process to pull out version information from JIRA
