Editorial issues in CSD candidate 1

1. Duplicated paragraph (lines 1940-1945 )
2. Incorrect references to Chapters 2,3,4,5 rather than Sections 2,3,4,5.
3. "A corollary to this is that subscribing to a Topic Filter at QoS 2 is equivalent to saying.." is vague, replaced with "Subscribing to a Topic Filter at QoS 2 is equivalent to saying.."
4. Several normative statements don't standalone (see lines 441, 613, 696, 1083, 1136 and 1264) need to add references to the appropriate Control Packets
5. line 707 normative statement MQTT-3.1.2.-5 is incorrectly numbered MQTT-3.1.2.-54
6. Appendix A does not match text in main body of spec
