      This issue applies in section 7.3.1 subheading "Index Scope" in all of ODF 1.0, ODF 1.0 ed.2 (IS 26300:2006), ODF 1.1, and ODF 1.2-1 CD04.

      The text:index-scope attribute is optional for the <text:table-of-content-source> element in all versions of ODF through ODF 1.2 Part 1 CD04.

      In no version of ODDF through ODF 1.2 Part 1 CD04 section 19.814 is a default value specified for this attribute.

      I suspect that the default is always "document" in reality, but it seems rather serious to not say so. The difference between "document" and "chapter" is non-trivial and would create dramatic interoperability issues if allowed to remain implementation-dependent.
