add source paragraph style to table of figures/objects/tables

      simply allow (optional) 8.11 <text:index-source-style> elements in the 3 corresponding text:*-index-source elements.
      If this elements exist, the designated paragraph styles will be used to find index entries; no update of the specification text should be necessary.

      Schema change is attached as 3-index-source-style.diff (the new one, unfortunately marking the old one as obsolete does not appear to be possible).

      The following 3 indexes don't currently allow creating from paragraph styles:

      text:illustration-index

      text:object-index

      text:table-index

      in OOXML it is possible (with limitations); in contrast to table-of-contents there is only a single level of the index and a single paragraph style is supported.
