This issue applies to all versions of ODF 1.0 up through ODF 1.2 Part 1 CD04. Although there is a modification in CD04, it is not clear and may not be accurate.

The defined values for the text:index-scope attribute are:

chapter: *the index is constructed for the chapter that contains the index. A chapter consists of all those paragraphs and headings which have
the same immediately preceding heading with outline level 1.*

document: the index is constructed for an entire document.
